Glaucoma, often referred to as “the sneak thief of sight,” is a disease that silently progresses without obvious symptoms. By the time you notice any vision loss, it may be too late to restore your eyesight. Why is glaucoma the main suspect?

Glaucoma involves damage to the optic nerve, which is responsible for transmitting visual signals to the brain. While the exact cause of this damage remains unknown, increased intraocular pressure (IOP) is a significant risk factor associated with glaucoma. When the optic nerve sustains damage due to high IOP, it can lead to incomplete transmission of signals from the eye to the brain. This can result in visual field loss and, if left untreated, eventually lead to blindness.

Why is IOP (Intraocular pressure) such an important clue?

The front of the eye contains a fluid called aqueous humour, which nourishes and bathes its various parts. Under normal conditions, this fluid flows out of the eye through different pathways and chambers. However, when these paths become blocked, the aqueous humour becomes trapped, causing a buildup of pressure and leading to high IOP.
Measuring IOP is a crucial diagnostic and treatment tool for glaucoma. Normal IOP ranges from 12 to 22 mm Hg. Tonometry is one of the most common tests used to measure IOP accurately. During this procedure, your doctor uses a tonometer to assess the pressure in your eye. While high IOP is a significant risk factor for glaucoma, it does not guarantee its presence, and normal IOP does not exclude the possibility of glaucoma. Controlling IOP is a primary goal in glaucoma therapy, as it reduces the risk of optic nerve damage and helps preserve vision.

Could I become a victim?

Glaucoma is a leading cause of preventable blindness, and approximately one in five individuals affected has a close relative with the condition. If you have any of the following risk factors, it is crucial to undergo regular eye checkups:
• Age over 40
• Family history of glaucoma
• High intraocular pressure (IOP)
• African, Asian, or Hispanic descent
• Previous eye injury or eye surgery
• Diabetes or hypertension
•  Nearsightedness or farsightedness
• Thin corneas

How can glaucoma be stopped?

While it may be challenging to completely halt glaucoma, extensive research demonstrates that early treatment plays a crucial role in preserving vision. The primary objective of glaucoma treatment is to lower intraocular pressure. Over the years, this approach has proven effective in preventing or slowing down vision loss in glaucoma patients. Medications and/or surgery can be used to lower IOP. Typically, medication is prescribed first, and surgery is considered for patients who do not respond adequately to or cannot tolerate medications. Most glaucoma patients manage their IOP levels by using prescribed eye drops. Laser treatment, such as laser trabeculoplasty, is a safe and painless option that can effectively treat glaucoma, especially for newly diagnosed cases. Successful laser treatment can maintain lower eye pressure for many years, eliminating the need for eye drops. Surgery is another option when eye pressure cannot be controlled by drops and laser treatment, or when patients cannot tolerate eye drops. It’s important to note that having glaucoma requires lifelong follow-up with an ophthalmologist. While the disease cannot be cured, proper management and regular checkups every six months can help control its progression.
